HealthWyse And Honeywell HomMed Partner To Deliver Real-Time Patient Monitoring To Home Care Agencies
Wilmington, MA - HealthWyse, a leading provider of home health and hospice agency automation software, announced today that it has partnered with Honeywell (NYSE: HON) HomMed, a world leader in telehealth and remote patient care, to fully integrate the LifeStream™ telehealth platform with the HealthWyse clinical patient record. The integration brings fragmented patient personal health data into one record that home health clinicians can use to make better informed care decisions.

The LifeStream platform is an open standards-based and web-enabled platform that provides clinicians, patients and family members with secure access to patient data. When combined with the HealthWyse clinical patient record, home care nurses have complete access to all patient information via a single application, thereby improving workflow processes and improving patient care. Patient information is available to home health nurses using HealthWyse's PalmWyse system in the field as well as to clinicians located in the agency offices.

AHCH's nurses now have access to complete patient records for more than 50 patients receiving home care, making their workflow and care decisions more efficient and well-informed. Additionally, the integration alerts caretakers in the event of an emergency, enabling them to intervene quickly and provide additional care when needed.
