Safety signals in special populations, such as pediatric patients, pregnant or lactating individuals, or cases involving medication errors or fatal outcomes, rarely show up clearly in standard clinical trial data. Because these situations are underrepresented in controlled studies, the earliest evidence of risk often surfaces in spontaneous reports, medical inquiries, literature abstracts, or quality complaints. When teams don't capture that information consistently or share it across teams, warning signs can go unnoticed until they become larger safety concerns.

This paper argues that reliable detection of these harder-to-spot signals depends less on any single detection method and more on how well an organization's functions work together.
