Refining The Patient Burden Index: A Data-Driven Approach To Protocol Optimization

Protocol complexity continues to rise, and with it, the burden participants feel throughout a trial's procedures, visits, and assessments. The Patient Burden Index (PBI) addresses this gap directly, offering a data-driven, objective way to quantify how demanding a protocol will feel for enrolled participants before the protocol is finalized. The PBI has grown and been substantially refined through a year-long effort grounded in literature review, cross-functional clinical expertise, and direct input from trial participants themselves.
The updated metric evaluates six core dimensions—pain, invasiveness, harmful exposure, duration, anxiety, and item count—scored across more than 4,000 clinical activities, including modern drug modalities like CAR-T and gene therapy, and the digital tools increasingly embedded in trial conduct. Results are now organized into clear Low, Medium, and High burden tiers, giving protocol designers an immediate, interpretable signal for where adjustments are needed. For teams designing protocols participants can realistically complete, this refined index offers a practical way to identify burden early and reduce the downstream risk of amendments and attrition.
